Abstract
Aggregation of platelets is the pathophysiologic basis of acute coronary syndromes. Eptifibatide binds to and inhibits the platelet glycoprotein IIb/IIIa receptor, the major platelet surface receptor involved in platelet aggregation.1 Eptifibatide is used as an adjunct to standard therapies for managing unstable angina or non-Q-wave myocardial infarction, which include aspirin, nitroglycerin, IV heparin, and ss-blockers. Some patients may require more aggressive management with angiographic evaluation and revascularization procedures (e.g., PCI, coronary artery bypass grafting, coronary artery stent implantation).3 In clinical trials, eptifibatide reduced the incidence of the composite endpoint of death or nonfatal myocardial infarction (MI) in patients with acute coronary syndromes who did not have persistent ST-segment elevation. In patients undergoing PCI, eptifibatide has been shown to decrease the rate of the combined endpoint of death, new MI, or need for urgent intervention. Eptifibatide is administered as an intravenous bolus followed by a continuous infusion. Common adverse events from therapy with eptifibatide include bleeding and hypotension.1
